These forms are a requirement for us to maintain proper client health records according to the College of Massage Therapists and our ability to provide a safer and more-effective treatment:

Clients who require treatment of 'sensitive areas':

​

If you present symptoms of low back pain, hip pain and/or sciatic-like nerve pain, or other discomfort or pain in a sensitive area of your body (ie. inner thighs, buttocks, chest), the CMTO requires clients fill out a Consent for Treatment of Sensitive Areas prior to treatment. Please print fill out the following form as indicated, sign, take a picture of it or scan and submit to findyourinnerzenca@gmail.com prior to your appointment.
